Program Management
DAC Program Director, John Luetmer, coordinates meetings for clients on an individual basis to ensure that programming is suitable and relevant to the client’s goals.
Meetings are held periodically (annually, semi-annually, etc.) and are attended by those directly involved in the client’s care.
Attendees may include housing staff, family members and/or guardians, as well as social service agency representatives, if applicable, and the client.
During meetings, specific client work abilities, task performance, and proficiency levels are outlined . . .
. . . and any situations and/or environments where the client needs to develop skills are identified.
Because each person attending the meeting serves the client in a different area of life (financial, housing, medical, etc.) . . .
. . . each can contribute information regarding changes experienced by the client since his or her last meeting and then adaptations to the current plan can be implemented (if necessary) to suit client needs.
Part of this plan is setting goals. Goals are individually tailored to help each client succeed in becoming an active member of the community.
